My name is Roymelle Jones-Mason, APRN, born and raised in Monroe, LA. I am a proud alumnus of Grambling State University where I received both my Bachelor's and Master's degrees in Nursing.
The Community NP, LLC is a practice of Nurse Practitioners who make house calls and telemedicine visits to homebound patients. TCN provides primary care services that are convenient and comprehensive to treat the whole patient. TCP was established to fill an unmet need for medical care among patients who are unable to leave their homes or have difficulty getting to their primary care provider. These patients have complex chronic conditions that require ongoing medical care. Our approach at TCN centers around providing ongoing primary care services as needed. This enables us to monitor patients' conditions and address problems before they become urgent so that we may 1.) Prevent unnecessary admissions to nursing homes 2.) Reduce the length and recurrence of hospitalization and 3.) Diminish unnecessary utilization of out-patient institutional services.
